Prior to senior living apartment admission, a number of forms must be completed. Please contact our administrator to make arrangements to complete the admission forms.

The following information is necessary to complete the admission process:

2. Social Security card
3. Birth certificate/ driver’s license
4. Resident’s birthplace
5. Attending physician’s history and physical report including name, address and telephone number
6. Dentist’s name, address, and telephone number
7. Responsible party’s name, address, and telephone number
8. Emergency party’s name, address, and telephone number
9. Pharmacy name, address, and telephone number
10. Advance directives (living will or durable power of attorney for health care)
