Admission Information

In order to ensure quality care as soon as your loved one is admitted to our facility, we would like to outline a few details to aid in the admission process.

Plan Ahead

Prior to admission, a number of forms must be completed. Please contact our administrator to make arrangements to complete the admission forms.

The following information is necessary to complete the admission process:

  1. Full name of resident
  2. Social Security card
  3. Birth certificate/ driver’s license
  4. Resident’s birthplace
  5. Attending physician’s history and physical report including name, address and telephone number
  6. Dentist’s name, address, and telephone number
  7. Responsible party’s name, address, and telephone number
  8. Emergency party’s name, address, and telephone number
  9. Pharmacy name, address, and telephone number
  10. Advance directives (living will or durable power of attorney for health care)

What to Bring

When moving in.

Streamside is an assisted living community that helps residents maintain a sense of dignity and independence in a warm, home-like atmosphere.

The apartments at Streamside are unfurnished, which allows each resident to express his or her own individuality. Suggestions for furnishings are: personal items such as toiletries, bed linens, bedspreads, pillows, and bath rugs; and furniture such as a bed, recliner chair, rocker, sofa or love seat, bookshelf, table and chairs, knick-knack shelf, dresser, chest of drawers, wall hangings, radio, television, computer, bedside stand and telephone. Please feel free to measure the new apartment when you make plans for placement of furniture and belongings.
